President
Neil Welch serves as President of Home Wellness Providers, LLC (HWP), where he spearheaded the creation and launch of HWP’s services. With a Doctorate in Pharmacy from the Massachusetts College of Pharmacy and Health Sciences and a pharmacist license spanning 24 states, Neil brings a wealth of clinical expertise and leadership experience to HWP.
Prior to his current role at HWP, Neil joined Injured Workers Pharmacy (IWP), HWP’s sister company, in 2018. At IWP, he played a pivotal role in enhancing clinical compliance programs and optimizing pharmacy workflows to elevate the patient’s experience. Most recently, he was instrumental in onboarding Advanced Wellness Pharmacy (AWP) into the Dignity & Wellness Providers (DWP) network. Before his time at IWP, Neil co-owned and operated an independent retail pharmacy in Massachusetts, fostering a deep commitment to patient-focused care.
With his extensive background in clinical operations and patient care, Neil is dedicated to driving HWP’s growth while delivering tailored, at-home monitoring and support services to complex, chronically ill patients. His vision is to create meaningful solutions that bridge the gap between patients and their care teams, ensuring health and wellness are maintained in the comfort of home.

Lara Bastajian is a dedicated Nurse Practitioner with significant experience in both primary and specialized healthcare environments. At HWP, she utilizes her comprehensive expertise in patient wellness, chronic disease management, and proactive health monitoring. While she has a strong passion for primary and chronic care as well as urgent medicine, she finds fulfillment in empowering her patients through effective chronic care management.
As a Board-Certified Nurse Practitioner and clinical practice leader at HWP, Lara brings over a decade of experience as a primary care provider, where she managed patients with complex chronic conditions before leading her own urgent care clinic for several years. This journey has exposed her to a variety of complex medical issues, enriching her clinical knowledge. Her robust background in primary care and urgent medicine has paved the way for a successful career as a Nurse Practitioner and a reliable healthcare provider, with a consistent focus on improving patient outcomes.

Michael Gavin joined Injured Workers Pharmacy as Chief Executive Officer in 2017. In January 2024, he became the CEO of DWP. As CEO, Michael leads the company’s efforts in providing uninterrupted prescription care to injured workers across the country. Michael has extensive experience in the insurance and medical treatment markets, providing strategic advisory services to hospital boards and medical staff and serving as the Vice President of Operations for a health services pay-for-performance start-up. Most recently, Michael served as the President of PRIUM, a medical peer review firm providing services to payers in the workers’ compensation and auto markets. Michael holds an M.B.A. from Emory University’s Goizueta Business School, as well as a B.S.B.A. degree in Finance from Georgetown University’s McDonough School of Business.

Jayne Kresac, Esq.
Vice President of Legal, Government Affairs and Communications
As the Vice President of Legal, Government Affairs and Communications, Jayne is General Counsel, handling contract negotiations, providing legal advice to the company and managing outside counsel. She is also responsible for creating DWP’s national legislative strategy as well as monitoring regulations and legislation that affect the DWP community. Jayne drafts and edits external and internal communications and oversees public relations strategies. Prior to joining DWP, she served as executive director of an Arlington, Virginia-based organization where she was responsible for the overall development, management, and administration of the firm’s programs to achieve the policy goals established by its board of directors. She has extensive state and federal legislative experience. Jayne earned her J.D. from Quinnipiac University and holds a B.S. in business from Babson College, and is admitted to the Massachusetts and the District of Columbia Bars.

Steve joined IWP in August 2018 as Controller before being promoted to Sr. Director of Finance and Controller in 2021. In January 2024, he became the CFO of DWP.
Steve has previously served as Global Controller of Software Revenue Recognition with Revvity (f/k/a PerkinElmer), Controller of ViaCord (from Revvity), and Controller at CyberGrants. Steve also held several positions with regional public accounting firms in the New England area. He holds a Bachelor of Science in Accountancy from Bentley University.

Pam joined IWP in 2012 as the Vice President of Strategy and Business Development. Pam now serves as Chief Operating Officer for Dignity and Wellness Patient Solutions, where she is accountable for ensuring resources are aligned to support the overall strategic direction of the business units. Functional areas include:
- Clinical operations and oversight to ensure best pharmacy practices to drive superior clinical care to our patients, improve operational efficiencies, and ensure the highest level of regulatory compliance
- Human Resources inclusive of recruitment, talent development and employee engagement
- Information technology and information security to protect the enterprise as well as constantly look for new opportunities to enable the business unit’s efficiency and effectiveness
- Revenue cycle and claims accounts receivables and cash collections management
- Operational quality assurance to ensure lifecycle management of employee performance
- Project Management Office (PMO)
Pam brings with her over 30 years’ experience in the workers’ compensation and insurance fields. Throughout her career, she has held diverse leadership roles in client relationship management, operations and underwriting with Sedgwick, Specialty Risk Services and The Hartford. She received a Bachelor of Arts, Management and Master of Business Administration from California State University, Fullerton.
